How to Register with the Surgery

To register with the practice you will need to complete a GMS1 Family Doctor Services Registration Form, which you can obtain from reception. You will also be asked to complete a health questionnaire form.

Newly registered patients are invited for a health check with the practice nurse.  You may express a preference regarding which doctor you would like to see.
